Access to medications
Not all pharmacies stock the often expensive and uncommon immunosuppressive treatments that transplant recipients require. Some recipients depend on the hospital where they had the transplant to carry these medications. In my personal experience, the hospitals sometimes have funding models for the medications that are only available if you receive the drugs through them. Make sure that you are prepared to provide your patients with access to their medications, or know where they can get access if you are not able to provide it for them.
Funding medications
Immunosuppressive treatments are expensive, and are likely to incur a significant financial burden on patients. As a pharmacist, you can provide them with information about the regional drug program such as the Trillium Drug Program in Ontario. The Kidney Foundation of Canada also provides some excellent resources regarding finances and how to receive tax deductions for medical expenses.
